This booklet has 25 pages stapled together probably made by midwife, Mary Bernarda Alishio Scavotto. The covers are beige with black binding which covers the staples. Each page has the heading "Physician's Record" followed by information on a birth. This includes the name of the mother and father and their place of birth, date and place of child's birth, and name of child. See image for a blank page giving the complete information. The pages often are not filled out completely and are written in pencil. At the top of some pages the amount paid or owed is written. On the inside of the front cover someone has written the names of drugs or remedies for various conditions. This booklet covers March 11-July 4, 1925. These are most likely babies that the midwife has delivered and the handwriting in most cases appears to be hers. No physician's name is given. There are approximately 25 of these booklets covering 1924-1942.

They went to St. Louis to be married and there Mary attended midwifery school graduating in 1905. The couple returned to Trinidad where they raised their family.
